Our providers and staff are committed to providing you with the best possible care. This commitment is demonstrated by our licensure by the Colorado Department of Health Services and Centers for Medicare and Medicaid Services (CMS). These organizations have established high standards for the practice and delivery of surgical services.

Hours of Operation
Our hours of operations are from 8 a.m. to 5:00 p.m., although we can open earlier and stay open later to accommodate you.

After Hours Calling
If you are calling after hours, please leave a message. Messages are checked first thing the following workday morning. We will do our best to return your call as quickly as possible. If you are calling regarding an health related emergency, please call 911.

Check List of Required Items for New Patients

  1. Photo ID or driver's license.
  2. Insurance card or worker's compensation information/authorization.
  3. Cash, check or credit card to pay for your co-payment or deductible requirements.

Injection and Surgery Policies

Procedure Day Policies

  1. Please bathe or shower before your surgery.
  2. Remove all make up. Do not use lotions or oils after bathing.
  3. Wear loose, comfortable clothing, such as sweat suits and easy-to-button shirts or blouses that fit extra large.
  4. Wear comfortable, flat-soled shoes.
  5. Leave all jewelry, dentures and contact lenses at home.

Before the Procedure

  1. Please arrange in advance for an adult to drive you home and remain with you the first 24 hours after surgery. You will not be allowed to have your procedure without arranging this responsibility.
  2. Please notify your surgeon before coming to the center if there is any change in your physical condition, such as a fever or cold.
  3. Be prepared to sign a form giving your consent for the procedure. If you are younger than 18, a parent or legal guardian must accompany you to sign the consent form.
  4. Plan to arrive promptly at your designated time.
  5. A nurse will provide you with a patient gown and a place to store your clothing before you are escorted to the pre-operative area. We will also check your temperature, blood pressure and pulse.
  6. Your family, friends and companions will be asked to wait in the front lobby. The surgeon will speak with your family immediately following the procedure.
  7. If you wear contact lenses, dentures or bridgework, please leave them at home. If you wear glasses, bring your case for their safekeeping.

Post Surgical Care Suggestions

? Before you leave the center, you will be given verbal and written instructions for your care at home. After you have returned home, please follow your doctor's orders regarding diet, rest, medication and activity. Unless directed otherwise, you should start your diet with clear liquids. Within four to six hours, you can begin to eat solid foods. By the next day, you should be back to your normal diet. You may feel a little sleepy, lightheaded or dizzy for several hours after your operation. Do not sign any important papers or make significant decisions for at least 24 hours.

? If you have any questions or concerns after your procedure, please contact your doctor immediately. A nurse from the center will call you within a few days after your surgery to see how you are recuperating and answer any additional questions you might have. Do not drive a car, smoke, drink alcoholic beverages, operate machinery or cook for at least 24 hours after returning home from your procedure.

 
Insurance and Financial Policies
 
Insurance
 
Insurance is a contract between you and your insurance company. Your doctor is contracted with most local insurance plans. Before any care is delivered, our staff will provide a complimentary benefits check to determine what services will be covered by your insurance.
 
Although we may estimate what your insurance may pay, it is the insurance company that makes the final determination of your eligibility.
 
You agree to pay any portion of the charges not covered by your insurance. If your insurance requires a referral and/or precertification, we will assist you, but it is your responsibility to make sure this has been done prior to the date of treatment.
 
Payment Options If You Have Insurance
 
  1. You can choose to pay your deductable and any out-of-pocket portions at the time services are rendered by cash, check, or credit card.
  2. If financial arrangement is needed, you may choose to pay 50% at the time of service and the balance in three weeks.
 
Payment Options If You Do Not Have Insurance
 
  1. You can choose to pay by cash, check, or credit card on the day that treatment is rendered.
  2. If financial arrangement is needed, you may choose to pay 50% at the time of service and the balance in three weeks.
  3. You may prefer to secure other third-party financing for the entire amount, and make payments to the lending institution. We are contracted with Care Credit, the nations leading third-party payor for healthcare services.
Monthly Statements
 
If you have a balance due on your account, we will send you a monthly statement.
 
Payments
 
The balance on your statement is due and payable when the statement is issued, and is past due if not paid withing 15 days, unless other arrangements are approved.
 
Charges to Account
 
We shall have the right to cancel your privilege to make charges against your account at any time. Future visits would then need to be paid for at the time of service.
 
Required Payments
 
Any co-payments required by an insurance company must be paid at the time of service. Because this is an insurance requirement, we cannot bill for these.
 
Past Due Accounts
 
If your account becomes past due, we will take necessary steps to collect this debt. If we have to refer your account to a collection agency, you agree to pay all the collection costs which are incurred, plus a late fee of $20 will be applied for missed payments.
 
Returned Checks
 
There is a fee (currently $30) for any checks returned by the bank. You agree to pay this fee plus the amount of the returned check by cash, money order, certified check, or credit card within 15 days.
 
Divorce
 
In case of a divorce or separation, the party responsible for the account prior to the divorce or separation remains responsible for the account. After a divorce or separation, the parent authorizing treatment for a child will be the parent responsible for those subsequent charges. If the divorce decree requires the other parent to pay all or part of the treatment costs, it is the authorizing parent's responsibility to collect from the other parent.
 
Waiver of Confidentiality
 
You understand that if this account is submitted to an attorney or collection agency, if your past due status is reported to a credit reporting agency, the fact that you have received treatment at this office may become a matter of public record.
 
 
Appointments & Responsibilities
 
Missed Appointment Fee
 
The third time a patient does not show up on time for an appointment, or cancels with less than 24 hours notice, a $20 fee may be charged. We will not file this fee to your insurance company; you are responsible. Patients with four missed appointments may be asked to transfer their records to another doctor.
 
Other Policies
 
Workers Compensation
 
In Workers Compensation cases, we require written approval/authorization by your employer and/or worker's compensation carrier prior to your visit. If your claim is denied, you will be responsible.
 
Co-Signature
 
In special circumstances a co-signature may be required. If this or another Financial Policy is signed by another person, that co-signature remains in effect until cancelled in writing. If written cancellation is received, it becomes effective with any subsequent charges.
 
If your insurance does not pay within 45 days, you will be required to start paying on your account. You will be reimbursed for any overpayments should your insurance company begin paying.
